You can also share your medical data with other health professionals and request formal copies of your records.<\/p>\n<\/div>\n                                  <\/div>\n    <\/div>\n  <\/div>\n\n\n<div class=\"paragraph-widget paragraph-widget--text-html\">\n\t<div class=\"idf-block text-content\">\n\t\t<h2>How to activate MyChart<\/h2>\n<p>Activating MyChart is easy and requires an activation code provided by your healthcare team. Once registered, you&#8217;ll have access to a powerful tool to manage your health, communicate with providers, and view your medical history anytime, anywhere.<\/p>\n\n\t<\/div>\n<\/div>\n\n\n<div class=\"paragraph-widget paragraph-widget--accordion idf-block accordion\" id=\"accordionFlush2216\">\n            <div class=\"accordion__accordion\">\n      <h3 class=\"accordion__title\" id=\"flush-heading_2216_1\">\n        <button class=\"accordion__trigger\" type=\"button\" data-bs-toggle=\"collapse\" data-bs-target=\"#flush-collapse_2216_1\" aria-expanded=\"false\" aria-controls=\"flush-collapse_2216_1\">\n          Automatic access to MyChart<span class=\"sp-before-icon\"><\/span>\n        <\/button>\n      <\/h3>\n      <\/h2>\n      <div role=\"region\" id=\"flush-collapse_2216_1\" class=\"accordion__content\" aria-labelledby=\"flush-heading_2216_1\" data-bs-parent=\"#accordionFlush2216\">\n        <div class=\"accordion__content-wrap text-content\"><p>If you already use MyChart through Piedmont Healthcare, you will automatically have access to your Shepherd Center MyChart. You can visit the <a class=\"\" title=\"\" href=\"https:\/\/mychart.shepherd.org\/shepherdprd\/Authentication\/Login\" target=\"_blank\" rel=\"noopener\">MyChart website<\/a> and enter your existing Piedmont log-in credentials. If you already use MyChart through another Epic organization, you must create a Shepherd MyChart account. Your accounts can be linked if desired.<\/p>\n<\/div>\n                                  <\/div>\n    <\/div>\n          <div class=\"accordion__accordion\">\n      <h3 class=\"accordion__title\" id=\"flush-heading_2216_2\">\n        <button class=\"accordion__trigger\" type=\"button\" data-bs-toggle=\"collapse\" data-bs-target=\"#flush-collapse_2216_2\" aria-expanded=\"false\" aria-controls=\"flush-collapse_2216_2\">\n          Outpatients will automatically receive a MyChart Welcome email to get started<span class=\"sp-before-icon\"><\/span>\n        <\/button>\n      <\/h3>\n      <\/h2>\n      <div role=\"region\" id=\"flush-collapse_2216_2\" class=\"accordion__content\" aria-labelledby=\"flush-heading_2216_2\" data-bs-parent=\"#accordionFlush2216\">\n        <div class=\"accordion__content-wrap text-content\"><p>When you schedule your next appointment with your Shepherd provider, a MyChart activation code will automatically be emailed to you if we have your current email address. Also, at the conclusion of your next outpatient appointment, you will be provided with an activation code that will allow you to register for MyChart. If you wish to receive an activation code ahead of your next outpatient appointment, you may call Shepherd Center\u2019s MyChart Support Team at <a href=\"tel:4044257250\">404-425-7250<\/a> or email <a class=\"\" title=\"\" href=\"mailto:mychartsupport@shepherd.org\">mychartsupport@shepherd.org<\/a> to request a MyChart activation code. <strong>Please provide the patient&#8217;s full legal name and date of birth in your request.<\/strong> After receiving this activation code, you will visit the MyChart website to set up your account and log in.<\/p>\n<\/div>\n                                  <\/div>\n    <\/div>\n          <div class=\"accordion__accordion\">\n      <h3 class=\"accordion__title\" id=\"flush-heading_2216_3\">\n        <button class=\"accordion__trigger\" type=\"button\" data-bs-toggle=\"collapse\" data-bs-target=\"#flush-collapse_2216_3\" aria-expanded=\"false\" aria-controls=\"flush-collapse_2216_3\">\n          Inpatients can also receive emails encouraging sign-up to their email on file<span class=\"sp-before-icon\"><\/span>\n        <\/button>\n      <\/h3>\n      <\/h2>\n      <div role=\"region\" id=\"flush-collapse_2216_3\" class=\"accordion__content\" aria-labelledby=\"flush-heading_2216_3\" data-bs-parent=\"#accordionFlush2216\">\n        <div class=\"accordion__content-wrap text-content\"><p>If you do not wish to have MyChart available during your admission, let staff know. Your case manager can assist you in setting up MyChart during your admission by providing an activation code and directions that will allow you to register. Download the MyChart two-factor authentication instructions.<\/p>\n<\/div>\n                                  <\/div>\n    <\/div>\n  <\/div>\n\n\n<div class=\"paragraph-widget paragraph-widget--text-html\">\n\t<div class=\"idf-block text-content\">\n\t\t<h2>Proxy forms<\/h2>\n<p>Complete a patient proxy form to allow authorized individuals to access a patient\u2019s MyChart account, ensuring seamless communication and coordination of care. This feature is designed to support caregivers, parents, or guardians in managing a loved one&#8217;s medical information securely and efficiently.<\/p>\n<h3>Instructions for proxy forms<\/h3>\n<p>You must be 18 years or older to register for MyChart. If you are younger than 18 years old, you will need to have your parent or guardian register on your behalf by completing the appropriate proxy form, which you can download in PDF format below.<\/p>\n<p>For various reasons, we also allow adult-to-adult healthcare proxies. You can download this form below, as well.<\/p>\n<p><strong>You should return the completed form to your Shepherd Center via one of these methods:<\/strong><\/p>\n<ol>\n<li>Use the <a class=\"\" title=\"\" href=\"https:\/\/securemail-shepherd.org\/s\/welcome.jsp?b=shepherd\" target=\"_blank\" rel=\"noopener\">Shepherd Center Zix Secure Email Portal<\/a> to securely email your attachment to <a href=\"mailto:mychartproxy@shepherd.org\">mychartproxy@shepherd.org<\/a>.
