At Shepherd Center, we&#8217;re committed to making this process easy and ensuring your information remains confidential. Whether you&#8217;re a patient, family member, or healthcare provider, we&#8217;re here to assist you.<\/p>\n<h3>How to request your records<\/h3>\n<ol>\n<li>Download and complete the <a href=\"https:\/\/shepherd.org\/wp-content\/uploads\/authorization-for-release-form.pdf\" target=\"_blank\" rel=\"noopener\">Authorization for Release of Information form (PDF)<\/a>.<\/li>\n<li>Fill out all necessary fields and sign.<\/li>\n<li>Submit:<\/li>\n<\/ol>\n<ul>\n<li>Fax the completed form to 404-350-7772.<\/li>\n<li>Mail the form to:<br \/>\nShepherd Center<br \/>\nRelease of Information<br \/>\nHealth Information Management<br \/>\n2020 Peachtree Road NW<br \/>\nAtlanta, GA 30309<\/li>\n<\/ul>\n<p><span style=\"color: #00649d; font-size: 1.9375rem; font-weight: bold; letter-spacing: 0px;\">Processing your request<\/span><\/p>\n<ul>\n<li><strong>Timeframe:<\/strong> We process requests in the order received. Please let us know if you need the information by a specific date, and we&#8217;ll do our best to accommodate you.<\/li>\n<li><strong>Fees:<\/strong> Georgia law allows for administrative and copying fees. For more information on fees, please call our Health Information Department at <a href=\"tel:404-350-7325\" target=\"_blank\" rel=\"noopener\">404-350-7325<\/a>. Fees are waived for copies sent directly to another healthcare provider for your ongoing care.<\/li>\n<\/ul>\n<h3>How to request patient images<\/h3>\n<p>If you are a patient who would like access to your medical images, download the <a href=\"https:\/\/shepherd.org\/wp-content\/uploads\/patient-powershare-account-guide.pdf\">How to Request Patient Images Guide<\/a> to help you request them through a Powershare account.<\/p>\n<h2>Need help?<\/h2>\n<p>Our Health Information Department is happy to answer any questions you have about obtaining your medical records.
