To help ensure a smooth transition, we&#8217;ve prepared a comprehensive list of the necessary information and documentation required before you\u2019re admitted into Shepherd Center.<\/p>\n<h2>Documentation and items needed before admission at Shepherd Center<\/h2>\n<ul>\n<li><strong>Health insurance information:<\/strong> Please provide a copy of your major medical health insurance card.
