This includes physical, occupational, and speech therapy, tailored to each patient\u2019s needs.<\/p>\n<h3>Patient satisfaction<\/h3>\n<p>When asked about the likelihood of their recommending Shepherd Center, patients or their caregivers rated Shepherd Center a 97 out of 100.<\/p>\n<h3>Where our patients go after discharge<\/h3>\n<ul>\n<li><strong>Community:<\/strong> 91%<\/li>\n<li><strong>Long-term care:<\/strong> 5%<\/li>\n<li><strong>Acute care:<\/strong> 3%<\/li>\n<li><strong>Hospice:<\/strong> 1%<\/li>\n<li><strong>Unplanned transfers:<\/strong> 2.6% of patients have an unplanned transfer to an acute care facility.<\/li>\n<\/ul>\n<h3>Self-care goals<\/h3>\n<p>63% of adult patients achieve or exceed their anticipated value of self-care upon discharge from Shepherd Center, compared to 62% nationally.<\/p>\n<h3>Mobility goals<\/h3>\n<p>65% of adult patients achieve or exceed their anticipated value of mobility upon discharge from Shepherd Center, compared to 62% nationally.<\/p>\n<p>Mobility scores tell us how much help a patient needs with activities like sitting, standing, walking, using a wheelchair, or going up and down stairs. A physical therapist helps patients get better at doing these activities on their own. If the scores go up from when the patient first came in to when they left, it means the patient got better at doing these activities independently.<\/p>\n<h3>Personal goals and progress<\/h3>\n<p>97% of adult patients reach or exceed their expected personal goals during their rehabilitation. Patients work with their therapy team to come up with goals. These goals may focus on physical or mental abilities that patients want to return to doing, or it can be something new. These improvements are measured using the Mayo-Portland Adaptability Inventory, which evaluates challenges to community reintegration and social and physical environments. Assessments are made at admission and again at discharge. SRS percentages for adult outpatients with brain injuries include:<\/p>\n<ul>\n<li><strong>Can be alone for 8+ hours:<\/strong>\u00a029%<\/li>\n<li><strong>Can be alone for up to 7 hours:<\/strong>\u00a011%<\/li>\n<li><strong>Other:<\/strong>\u00a060%<\/li>\n<\/ul>\n<p>The category of \u201cother\u201d represents varying supervision levels, such as:<\/p>\n<ul>\n<li><strong>Patient can be left alone for up to 1 hour:<\/strong>\u00a0Supervision is provided to the patient overnight &amp; part-time during waking hours. Supervising caregiver cannot work a full-time job.<\/li>\n<li><strong>Full-time indirect supervision:<\/strong>\u00a0Patient is never left alone. Supervising person checks in 30 minutes or less.<\/li>\n<li><strong>Full-time direct supervision:<\/strong>\u00a0Patient is never left alone. Supervising person checks in more than every 30 minutes.<\/li>\n<li><strong>Physical control of exits:<\/strong>\u00a0Patient lives in a setting in which exits are physically controlled by others, plus a supervising person is designated to provide full-time line-of-sight supervision.<\/li>\n<li><strong>Physical restraints:<\/strong>\u00a0The patient is in physical restraints.<\/li>\n<\/ul>\n<h3>Personal goals and progress<\/h3>\n<p>68% of adult outpatients reach or exceed their expected personal goals during their outpatient experience.
